AHRQ’s research contributed to the development of a Chronic Disease Self Management Program (CDSMP) that is having a positive impact on the quality of life of an aging population. It also saves health system resources. One CDSMP project, undertaken by Kaiser Permanente, produced nearly $5 dollars in avoided hospitalizations and ER visits for every $1 spent on program costs.
